HOW TO ENTER: To enter, log on to www.davidsprom.com, fill in the on line registration form and submit it in accordance with the on screen directions during the sweepstakes period and you will be entered once in the sweepstakes. More than one entry from any person or email address will void all entries from that person or email address. To enter without participating in the online registration, send in a 3" x 5" card, with hand printed name, address, age and daytime phone number (with area code) to "Win A Free Prom Dress", c/o David's Bridal, 1001 Washington Street, Conshohocken, PA 19428, before May 15, 2007. One entry per person. David's Bridal is not responsible for lost, late, mutilated or misdirected mail. Entries that are illegible, incomplete, or otherwise not in compliance with these official rules will be disqualified. The sweepstakes entry period begins December 13, 2006 and ends May 15, 2007. 2. ENTRY LIMITATIONS: Only one entry per person. Open only to legal residents of the 50 United States, Puerto Rico and the District of Columbia, 13 years of age or older, who are not employees of David's Bridal, their subsidiary, parent or affiliated companies, nor members of their immediate families. In the event of a dispute over the identity of an online entrant, entry will be deemed submitted by the holder of the e mail account. Use of automated devices not valid for entry. Sponsor not responsible for error, omission, interruption, deletion, defect, delay in operations or transmission, theft or destruction or unauthorized access to or alteration of entry materials, or for technical, network, telephone equipment, electronic, computer, hardware or software malfunctions of any kind, or inaccurate transmission of, or failure to receive entry information by sponsors on account of technical problems or traffic congestion on the Internet or at any web site or any combination thereof. Sponsor is also not responsible for any injury or damage to entrant's or any other person's computer related to or resulting from participating in or downloading any materials in the sweepstakes. If for any reason this sweepstakes is not capable of running as planned, including, but not limited to, infection by computer virus, bugs, tampering, unauthorized intervention, fraud, or any other causes beyond the reasonable control of sponsor which corrupt or affect the administration, security, fairness, integrity or proper conduct of the promotion, then Sponsor reserves the right to at its sole discretion to cancel, terminate, modify or suspend the sweepstakes. 3. LIMITATIONS: Void were prohibited or restricted by law and subject to all laws and regulations. All entries become the Sponsor's property and will not be returned. By entering this sweepstakes, each entrant agrees to be bound by these rules and the decisions of the judges. Acceptance of prize constitutes the grant of an unconditional right to use the winner's name and hometown, voice and/or likeness for any and all publicity, advertising and promotional purposes without additional compensation, except where prohibited by law. Sponsor and their related, affiliated parent and subsidiary companies are not responsible for claims, injuries, losses or damages of any kind resulting from participation in this sweepstakes or the acceptance, use, misuse, possession, loss or misdirection of any prize. 4. WINNERS: All reasonable efforts will be made to contact the winner. All responsibilities of a minor who is a winner will be assumed by the minor's parents/legal guardian. Winners are required to prove eligibility and will be required to sign an affidavit of eligibility and release. The failure of a potential winner to verify address, or the return of notification as undeliverable, or the failure to return the signed affidavit and release will result in disqualification and the selection of an alternate winner. In the event of any other noncompliance with rules and conditions, the prize may be awarded to an alternate winner. All expenses not specifically included, are the sole responsibility of the winner. WA and VT residents may omit return postage. 5. PROCEDURES: In a random drawing conducted on a monthly basis, one (1) winner will be chosen from all eligible entries. Odds of winning depend on the number of eligible entries received during the prior month. 6. PRIZES: One (1) winner per month will receive the following prize carried by David's Bridal: (1) Prom Dress. Alterations not included. All bridal gowns excluded. Prize is not redeemable for cash or transferable; and no substitutions allowed except at the discretion of the Sponsor, who may substitute prizes of equal or greater value. Winner is responsible for all taxes. Prize is based on current retail value. 7. SPONSOR: David's Bridal located at 1001 Washington Street, Conshohocken, PA 19428. Right of Non-participation: Pursuant to federal law, any individual has the right to notify Sponsor that he/she does not wish to be contacted by the Sponsor regarding any future sweepstakes, contests or promotions. The Sponsor may be so notified by mail at David's Prom 2004, Attention: Marketing Department, 1001 Washington Street, Conshohocken, PA 19428, or by sending an email to remove@davidsbridal.net. 9. All entrants must provide name and mailing address. Additional information is optional. Before providing any such information, please read our Privacy Policy. which is available on David's Bridal's website. By submitting a completed entry form, you consent to the collection, processing, use and disclosure of the information contained in your entry form according to our Privacy Policy.
